What is Home and Community Based Services (HCBS)?

Home and Community Based Services (HCBS) is a Medicaid program that supports your right to live, work, and take part in your community.

These rules help make sure you have choices about your life, receive services in community settings, and are treated with dignity and respect. HCBS supports you in living where you choose and participating fully in your local community.

How this program works:

Being Part of Your Community
You have the right to take part in community activities, visit local places, and build relationships. We support you in doing the things that matter most to you.

Living in Community Settings
Services are provided in community settings that support independence, privacy, and personal choice.

A Plan Built Around You
We use person-centered planning, which means you guide the process. We listen to your goals, preferences, and strengths to create a plan that reflects the life you want to live.
